Introduction of Eslify
Eslify (ESL) is a new antiepileptic drug (AED) approved for use in adults with partial-onset seizures by the US Food and Drug Administration (FDA). It is a prodrug of the active metabolite oxcarbazepine (OCB), and is structurally similar to it. ESL is a second-generation AED that has proven efficacy and tolerable side effects.
Uses for Eslify
Eslify is indicated for the treatment of partial-seizure in adults 18 years and older. It is a once daily medication and hence it is suitable for long-term maintenance treatment for partial-seizure in adults.
Mechanism of Action of Eslify
Eslify works by prolonging the inactive state of opioid receptor channels and blocking the voltage-gated sodium channels in an incomplete manner, which prolongs the cortical and subcortical inhibitory neurons. This reduces the irritability and excitability in the brain and helps in suppressing the abnormal neuronal activities.
How Long Does It Take To Work?
The peak effect of Eslify is seen within 1-2 weeks. It may take up to 8 weeks before the full effect is seen.
Absorption of Eslify
Eslify is rapidly and almost completely absorbed following oral administration. It has a bioavailability of 61% and exhibits linear pharmacokinetics.
Route of Elimination of Eslify
Eslify is mainly eliminated through faecal route (65%). It undergoes metabolic transformation which is followed by excretion via urinary route (35%).
Dosage of Eslify
The recommended starting dose of Eslify is 200mg once daily and the common maintenance dose is 800mg once daily. The maximum dose is 800mg once daily. Elderly patients may require a lower dose depending on their condition.
Administration of Eslify
Eslify should be taken once daily with or without food. It should be taken at the same time each day.
Side Effects of Eslify
Some common side effects of Eslify include: drowsiness, nausea, vomiting, headache, dizziness, constipation, blurred vision, nystagmus, and tremor.
Toxicity of Eslify
In case of overdose, monitor patients for signs of toxic effects. Symptoms of overdose include metallic taste, agitation, and vomiting.
Precautions of Eslify
Eslify should be used with caution in patients with hepatic and renal impairment. Usage of this drug should be avoided in pregnant and breastfeeding women. Avoid operating machinery or driving.
Interaction of Eslify
Eslify may interact with other drugs like diazepam, theophylline, valproic acid, nitrazepam, phenytoin, and lidocaine so should not be used with any of these drugs.

Eslify may interact with some diseases like kidney and liver diseases, psychotic disorders, depression and cardiac disorders. In such cases, dose adjustment may be necessary.
Drug Interactions of Eslify
Eslify may interact with drugs like protease inhibitors, cimetidine, methadone, phenytoin, valproic acid, phenobarbital, and carbamazepine. It is important to inform the doctor of all the drugs being taken before starting ESL.
Food Interactions of Eslify
Eslify should not be taken with grapefruit or grapefruit juice as it may increase its level in the blood and lead to side effects. Patients should also avoid alcohol while taking ESL.

Eslify should not be used during pregnancy unless the potential benefits outweigh the risks. Patients should inform the doctor if they plan to become pregnant or if they are pregnant.
